Who it's for
Anyone whose erections have become unpredictable and who suspects performance anxiety is part of it, working alongside a medical provider.
Draws on Masters and Johnson (sensate focus and spectatoring), David Barlow's model of anxiety and sexual response, and recommended reading from McCarthy and Metz, Bernie Zilbergeld, and Aaron Spitz, MD. Educational only; My Mental Climb is not affiliated with these authors, and this is not a substitute for medical care.
My Mental Climb adapts established therapy frameworks for educational use and is not affiliated with, endorsed by, or certified by their originators, including The Gottman Institute.

FAQ
Can therapy help with erectile dysfunction?
+
Yes, therapy can help erectile difficulties when performance anxiety, stress, or relationship factors are part of the picture, and it works best alongside a medical evaluation. CBT and sex therapy target the anxiety loop that makes erections unpredictable, using cognitive restructuring, reducing self-monitoring during sex, and sensate focus. Because erections also have physical causes, a urology or men's health workup should come first.
Is erectile dysfunction psychological or physical?
+
Erectile difficulties can be physical, psychological, or both, which is why a medical workup comes first. A useful clue: if erections show up on your own or on waking but not with a partner, performance anxiety is likely a large part of it; if they are constant, a physical cause is more likely. Most people benefit from addressing both layers together.
What CBT techniques help with erection difficulties?
+
CBT for erection difficulties targets performance anxiety through a few core tools: catching and rewriting the catastrophic thought, stopping spectatoring (watching yourself during sex), sensate focus that takes intercourse off the table so the body can respond without pressure, and slowing the nervous system. This handout walks through each with fill-in worksheets.
